The mission of Blossom Tree Healthcare LLC is to provide accessible, recovery-focused behavioral health services that empower individuals and families affected by substance use disorders to achieve lasting wellness. We deliver ethical, person-centered care in a safe and supportive environment using evidence-based practices and collaborative treatment planning.

Who We Are

Blossom Tree Healthcare LLC is a legally organized and operated limited liability company and a state-licensed Alcohol and Other Drug Entity (AODE) in Kentucky. We provide evidence-based outpatient and intensive outpatient (IOP) treatment services for adults with substance use disorders, with a commitment to improving client health outcomes and reducing health disparities within the communities we serve.

We operate in compliance with 908 KAR 1:370, offering structured outpatient care aligned with ASAM Criteria, including Level 1.0 Outpatient Services and Level 2.1 Intensive Outpatient Programs, designed to meet varying levels of recovery needs.

Our services include individual, group, and family counseling, comprehensive assessments, and personalized treatment planning using evidence-based approaches such as Cognitive Behavioral Therapy (CBT) and Motivational Interviewing (MI). Services are provided in person across Jefferson, Oldham, Shelby, Henry, Trimble, Fayette, and Bullitt Counties, as well as through statewide telehealth services across Kentucky, ensuring accessible and flexible care for those seeking recovery.
